History of Gold Thread Implantation

The technique of gold thread implantation in cutaneous areas which are flaccid due to aging was developed in France about 30 years ago by Caux and was improved by Parisot 8 years ago. many kinds of materials, such as simple catgut, composed catgut, and synthetic threads, were utilized to promote skin support. However, gold was considered safer because of its purity, its density, its color and mallcability, and also due to its being an inert materials. The only study done on this technique based on histopathology was done by Orenes on mice using catgut and gold thread implantation. A granulomatous reaction was observed with catgut threads and a granulomatoid reaction with gold threads.

Improve the skin color and reinforce loose skin

The results of the present study indicate that the reinforcement of facial skin undergoing age-specific involutional changes with gold surgical filaments provides a powerful tool with which to achieve a conspicuous cosmetic effect. The differential choice of surgical modalities taking into consideration the severity of involutional changes in individual patients ensures good and satisfactory aesthetic outcome in the majority, disappearance of wrinkles associated with grade 1 ptosis, their smoothing in patients with grade 2-3 skin ptosis, and partial correction in those with grade 4 involutional changed in the facial skin.

Absence of tissue rejection at the implant site

A granulomatoid reaction of the tuberculoid type, enveloping the gold thread with a large quantity of reticulin fibers and characterized by the presence of epithelioid cells and the absence of giant multinuclear cells, was observed. It demonstrated the absence of foreigh-body reaction and the absence of tissue rejection at the implant site.

Improvement microdirculation

Doppler fluorometry has demonstrated an enhanced microcirculation index(MI) in the immediate postoperative period(a 23-29% rise in all the patients). The microdirculation index remained elevated 2-3 months after the treatment, thouht it was lower(21-26% above the baseline value) than 4-7 days postoperatively. This enhancement persisted over 5-6 months of follow-up.
